he header of the second stream is broken.
This often happens during crossflashes from 4.1 to 5.1 or after crashes. Those crossflashes have other issues as well, btw.
You can try a Full NVRAM Reset: Boot with the keys (cmd-alt-p-r) together continuously without pause until the Mac Pro chimes 4 times.
After this procedure the NVRAM is zapped and you may repeat some settings like blessing the bootloader, etc.
If the problem repeats, I recommend rebuilding the bootrom.
